In order to apply for a Czech visa you are required to include certain documents in your application. Be aware that these always have to be legalised.

All documents given to the embassy or consulate either have to be originals or certified copies. Any non-Czech document has to be legalised, using an apostille, for instance. The documents have to be issued within the last six month with the exception of passports and documents that are older by nature, such as birth certificates.

Moreover, documents have to be in Czech or officially translated into Czech. This has to be done by officially accepted translators. Your embassy will be able to provide contact details.
